- Use appropriate assessment tools and techniques to identify patient needs and develop care plans.
- Execute and continuously monitor care plans to ensure patient safety and well-being.
- Engage patients and their families in the care process, building rapport and trust through a collaborative approach.
- Maintain and update patient records, ensuring accurate and timely documentation.
Requirements:
- Registration with the CNO as a Registered Practical Nurse in good standing.
- Valid driver's license and access to a vehicle.
- Experience in delivering community care is an asset.
- Reliability, flexibility, and a passion for making a difference in your community.
- Willingness to travel to and from patient residences within a designated area.
- Ability to share in on-call coverage to ensure patients receive care during their time of need.
