About the Role

We are seeking a highly skilled and dedicated Social Worker to join our multidisciplinary team at the Blackheath Brain Injury Rehabilitation Unit in London.

Key Responsibilities
  • Manage a defined caseload of patients, ensuring the delivery of high-quality, individualized care.
  • Develop and implement evidence-based packages of care, including psychosocial interventions as needed.
  • Work in partnership with service users, carers, and other agencies to ensure seamless transitions and optimal outcomes.
  • Conduct assessments, care planning, and record-keeping in accordance with relevant policies and procedures.
  • Collaborate with the multidisciplinary team to provide comprehensive care and support.
