Empowering Young People to Thrive

Hays Social Care is seeking a dedicated and compassionate Leaving Care Support Worker to join our team in South Wales. As a key member of our team, you will play a vital role in supporting young people aged 16+ as they transition from care to independent living.

Key Responsibilities:

  • Provide practical and emotional support to young people, helping them develop the skills they need to succeed.
  • Work closely with social workers, educators, and other professionals to create and implement personalized support plans.
  • Travel around South Wales to meet different services, providing support and guidance to young people.

Requirements:

  • Experience or qualifications within social care or education are highly beneficial.
  • A valid driving license and willingness to travel across South Wales (mileage is paid).
  • Ability to build trusting relationships with young people.
  • Knowledge of the challenges faced by care leavers and strategies to support them.
