About the Role

We are seeking a skilled and compassionate Community Healthcare Assistant to join our Rapid Response & Intermediate Care (RRIC) team in Buckinghamshire. As a key member of our team, you will provide high-quality nursing, social, and rehabilitative care to patients in their own homes, promoting independence and well-being.

Key Responsibilities
  • Deliver person-centered care to adults with complex needs, working collaboratively with senior staff and other healthcare professionals.
  • Take responsibility for designated elements of patient treatment protocols, ensuring timely and effective delivery of care.
  • Contribute to the development of innovative care plans, utilizing your expertise to improve patient outcomes.
  • Work effectively with community teams to ensure seamless service delivery and high-quality patient care.
  • Participate in ongoing training and development to enhance your clinical expertise and skills.

Requirements

To be successful in this role, you will need:

  • Previous experience in a healthcare or social care environment.
  • Awareness of the needs of older people and those with long-term conditions.
  • Good interpersonal and communication skills, with the ability to work effectively in a team.
  • Valid and current driving license and access to a car for work purposes.
