About the Role

We are seeking a skilled and compassionate Healthcare Assistant to join our team in the Community Crisis Support Service. As a Healthcare Assistant, you will play a vital role in providing short-term crisis support to individuals in their homes, helping to prevent hospital admissions.

Key Responsibilities:

  • Assess clients' needs and develop personalized care plans.
  • Provide support with personal care, medication prompts, end-of-life care, and emotional support.
  • Work collaboratively with health and social care professionals to ensure seamless care delivery.

Requirements:

  • Educated to GCSE level (or equivalent by experience).
  • Ability to communicate effectively with clients and healthcare professionals.
  • Knowledge of supporting individuals with disabilities and/or frailty.

What We Offer:

  • A competitive salary of £22,625 (pro rata for less than 35 hours).
  • A comprehensive benefits package, including 36 days holiday (including Bank Holidays) per annum, pro rata.

Location: Community based (Rushcliffe, Nottingham East and West).

Working Hours: 35 or 30 hours per week, with flexible shift patterns to accommodate your needs.
