Position Overview: The successful candidate will play a crucial role in delivering a high-quality, time-sensitive assessment and brief intervention service aimed at enabling clients to achieve optimal functioning within their communities.
Key Responsibilities:
- Deliver interventions utilizing an integrated Motivational Interviewing approach that is both person-centered and holistic.
- Conduct assessments and provide support for families and/or caregivers, collaborating effectively with other Oxleas services and substance misuse providers.
- Work autonomously within a multi-disciplinary team to ensure high-quality assessments and responsive communication.
- Provide professional supervision and leadership to all clinical staff involved in COMHAD and commissioned substance misuse services.
Goals:
- Enhance communication and information sharing between Oxleas NHS Foundation Trust and Substance Misuse Service Providers regarding COMHAD-related acute admissions.
- Strengthen collaborative protocols for discharge planning between Oxleas NHS Foundation Trust and Substance Misuse Services.
- Increase engagement with Substance Misuse Services for individuals with drug and alcohol dependence currently hospitalized in mental health acute wards.
- Reduce the frequency of repeated mental health acute admissions related to substance misuse.
- Minimize reliance on acute mental health beds due to substance misuse factors.
- Improve the health and social care pathways for dual diagnosis service users by enhancing their engagement with appropriate services.
- Offer consultation services to all Oxleas mental health teams involved in acute, crisis, and liaison mental health services.
