Job Summary

This is an exciting opportunity to join an established Community Nursing workforce and be part of the Dumfries South Home Team. As a Senior Practitioner, you will play a key role in delivering high-quality patient care and contributing to the development of the team.

Key Responsibilities
  • Manage a caseload of complex patients and coordinate care with other healthcare professionals.
  • Make clinical decisions and take accountability for patient outcomes.
  • Work closely with the Home Team, including Social Work, Public Health, and Allied Health Professionals.
  • Develop and implement care plans that meet the needs of patients and their families.
  • Contribute to the development of the Community Nursing Service and participate in quality improvement initiatives.
Requirements
  • Full UK NMC registration.
  • Ability to travel across the area.
  • Qualifications in V300 independent and supplementary prescribing and advanced clinical assessment.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
