Adult Social Worker for Transition Support

We are seeking an experienced and skilled Adult Qualified Social Worker to join our Transitions Team. As a Social Worker in this role, you will work with young people transitioning from children's services to adult services, assessing their needs and providing support during this critical phase.
Key Responsibilities:- Conduct assessments under the Care Act 2014 to identify the needs of young people transitioning to adult services.
- Develop personalized care plans that meet the changing needs of young people and reflect good value for money.
- Facilitate access to services that promote independence, well-being, and long-term support for young people.
- Collaborate closely with young people, their families, and multidisciplinary teams to ensure a smooth transition into adult services.
- Monitor, review, and maintain up-to-date support plans to ensure they meet the changing needs of young people.
- Advocate for young people's rights, choices, and access to services.
- Examine and supervise carer strategies to ensure they are effective in supporting young people.
- Safeguard and promote the welfare of young people, adhering to relevant legislation and policies.
- 2 years of experience post-qualification in a social work role.
- Experience in conducting assessments and developing care plans.
- Knowledge of the Care Act 2014 and relevant legislation.
- Ability to work collaboratively with young people, families, and multidisciplinary teams.
