They will work collaboratively with the wider multidisciplinary team to ensure that all health and social needs are met, and contribute to meeting performance targets for the organisation.
RequirementsThe ideal candidate will have experience working with adults with learning disabilities, and will be able to recommend appropriate interventions to promote rehabilitation, independence, and safety. They will also be able to maintain effective multidisciplinary communications and ensure safe, appropriate, and effective provision of equipment and services.
