About the Role

We are seeking a skilled Social Worker to join our Young People with Disabilities Team, providing assessment, advice, and support to children and young people with disabilities and their families.

Key Responsibilities
  • Manage a caseload of children and young people with disabilities, providing assessment of their needs and developing care plans and support packages.
  • Build relationships with young people and families using systematic and strengths-based social work approaches.
  • Work alongside young people and their families during the transition from Children's services into adulthood, including support into accommodation, day services, and employment.
  • Undertake Transitions/Pathway Assessments/plans, Mental Capacity Assessments, Community Care assessments, and implementing and reviewing support packages/plans.
Requirements
  • Diploma or degree in social work, CQSW, CSS or equivalent.
  • Registration with Social Work England as a registered Social Worker.
  • Demonstrable capability of practice in accordance with current professional standards.
  • Ability to use information technology and related systems, including word and social work case recording systems.
  • Flexibility to work as required, including evenings and weekends.
