Community Nursing Specialist
The Sussex Community NHS Foundation Trust is seeking dedicated registered nurses to enhance our Community Nursing Team.
We currently have an opening for a Band 5 Community Nursing Specialist. Whether you are newly qualified or seeking a fresh opportunity, community nursing presents a unique chance to specialize in a dynamic and fulfilling area of healthcare.

Community nursing encompasses a variety of specialties within a single role.
We welcome applications for both part-time and full-time positions as Band 5 Community Nursing Specialists.
Our service operates from 8 AM to 6 PM, with flexible working hours available within this timeframe, including a commitment of 1 in 4 weekends.
Successful candidates will receive a personalized induction plan to facilitate their transition into the community setting.
Key Responsibilities
Our team is composed of administrators who support the nursing staff, healthcare assistants, nursing associates, and community staff nurses, both part-time and full-time. We also have qualified district nurses and community sisters with specialized training in community nursing, along with clinical nurse leads and a team lead.
Primary Duties
As outlined in the Person Specification, candidates should possess or be willing to develop the competencies necessary for this role. The core responsibilities include:
- Diabetic Management
- Wound Management, including diagnosis and treatment of leg ulcers
- Palliative care in collaboration with hospices and general practitioners
- Bladder and bowel care, including management of urethral and suprapubic catheters
All registered staff are expected to support junior staff and learners, including trainee nurse associates and nursing students.
In addition to traditional patient care, we collaborate with local health and social care providers to support a specific demographic, including:
- Severely frail individuals
- Patients with dementia
- End-of-life care
- Complex conditions and care home residents

Job Responsibilities
We adopt a supportive approach to help all staff acquire the necessary competencies for their roles. - Registered Nurse Qualification (Degree or Diploma)

- Clear understanding of the Community Nursing Role
- Ability to promote independence and choice in patient care
- Knowledge of Clinical Governance, Clinical Audit, Clinical Supervision, and NMC Professional Code of Conduct
- Familiarity with current SCFT Policies regarding Community Health and Social Care Services
- Proficient in IT
- Strong numeracy skills

- Demonstrated competency in various skills, including medicine management and administration, palliative/end-of-life care, and syringe driver use
