Nurse Case Manager Job Description

Job Title: Nurse Case Manager

Location: Merseyside

Salary: Up to £38,000 depending on experience

Contract: Full-Time, Permanent

At Routes Healthcare (North), we are seeking a highly skilled and dedicated Registered Nurse to join our team as a Nurse Case Manager. As a key member of our healthcare team, you will play a pivotal role in delivering high-quality patient care and making a positive impact on the lives of our patients.

Key Responsibilities:

  • Provide comprehensive case management services to patients with complex healthcare needs
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and communication
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies
  • Monitor patient progress and adjust care plans as needed to ensure optimal outcomes

Requirements:

  • Registered Nurse qualification with current registration with the NMC
  • Proven experience in case management or a related field
  • Excellent communication and interpersonal skills
  • Ability to work autonomously and as part of a team
  • Strong problem-solving and analytical skills
