About the Role
We are seeking a highly skilled and compassionate Registered Nurse to join our Community Staff Nurse team at West Mendip hospital in Glastonbury. As a Community Nurse, you will provide high-quality care to patients in their homes, managing complex health issues and collaborating with other healthcare professionals.
Key Responsibilities
- Provide holistic assessments of patients' care needs and develop individualized care plans
- Deliver high-quality care to patients in their homes, including IV medication administration, palliative and end-of-life care, and wound management
- Collaborate with other healthcare professionals to ensure comprehensive care and support for patients
- Develop and maintain strong relationships with patients, carers, and families to ensure person-centered care
- Work autonomously and as part of a team to achieve high-quality care outcomes

Person Specification
- Appropriately registered with the Professional body as per post requirements
- Relevant degree in Nursing or equivalent qualification
- Knowledge of current national policies affecting the NHS
- Ability to communicate effectively with patients, carers, and families
- Compassionate and empathetic approach to patient care
