We seek a dedicated, compassionate, and proactive individual with outstanding clinical, organizational, and communication skills. The successful candidate will serve as a Clinical Pharmacist Practitioner, collaborating with healthcare professionals and contributing to multidisciplinary teams.
In this role, you will provide comprehensive care, including assessment, diagnosis, and treatment of patients. You will also facilitate referrals and promote self-management among patients, empowering them to make informed decisions regarding their healthcare.
