Job Summary:
We are seeking a highly skilled and compassionate Support Worker to join our team at Home Care Direct. As a Support Worker, you will play a vital role in providing high-quality care and support to our clients, enabling them to maintain their independence and live fulfilling lives.
Key Responsibilities:
- Provide personal care and support to clients, including assistance with daily living activities, mobility, and communication.
- Assist clients with medication management, including administration and monitoring.
- Support clients with their daily routines, including meal preparation, cleaning, and laundry.
- Assist clients with their social and recreational activities, including outings and appointments.
- Monitor and report any changes in client condition or behavior to the care team.
- Work collaboratively with the care team to develop and implement personalized care plans.
- Maintain accurate and up-to-date records of client care and support.
Requirements:
- Previous experience in a care or support role, preferably with a focus on physical disabilities.
- Ability to work independently and as part of a team.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
- Valid driving license and access to a vehicle.
- Enhanced DBS disclosure.
