Job Summary

The Market Surgery is seeking a highly skilled and experienced Nurse Team Leader to join our team. As a key member of our nursing team, you will be responsible for leading and supervising our nursing staff, providing safe and effective care to our patients, and contributing to the development of our nursing services.

Main Responsibilities
  • Lead and supervise the nursing team, ensuring that high standards of care are maintained and that the team is working efficiently and effectively.
  • Develop and implement new clinical services and initiatives, in collaboration with the General Practitioners and practice manager.
  • Provide clinical care for practice patients, including chronic disease management, wound care, and health promotion.
  • Implement new clinical services after agreement by the practice.
  • Act as the practice lead for better working with other nursing services providing care for our patients.
Leadership and Management Responsibilities
  • Attend practice/primary care meetings, updating the partners and management with relevant information.
  • Have regular meetings with the nursing team to understand any clinical or organizational problems.
  • Work closely with the practice manager to manage staff within the nursing team.
  • Participate in the recruitment of new staff when the need arises and ensure that they are inducted fully into the practice.
  • Work with management to ensure that all staff have a training plan and keep themselves professionally updated.
Clinical Practice
  • Practice independently in accordance with UKCC code of conduct and with reference to practice guidelines, appropriately referring to GPs and other professionals.
  • The work of the team includes:
    • Chronic Disease Management:
      • Working with other healthcare professionals and in line with policy, to diagnose, monitor, and manage patients with:
      • Diabetes
      • COPD
      • Hypertension
      • CHD
      • Asthma
      • Obesity
    • Wound Care and Management:
      • Assessing routine wounds including trauma.
      • Assessing to support good wound management in complex situations.
    • Prevention and Health Promotion:
      • Working with patients to develop a management plan where health problems or potential health problems are identified.
      • Identifying health trends and public health issues and working with the team and others to combat these and promote increased physical activity, healthy eating, smoking cessation, safe alcohol drinking, and sexual health.
    • Risk Assessment:
      • Recognizing issues and gathering sufficient information to refer (e.g. drugs; domestic violence; child protection; vulnerable adults; social problems).
    • Health Screening:
      • Ensure the Practice delivers on its role as part of the national screening program.
    • Travel Health:
      • Provision of specialist and evidence-based support to the team.
    • Immunisation (Adult and Child):
      • Working with public health initiatives ensuring the work of the team is up to date.
      • Leading the team in achieving the immunisation guidelines and targets.
    • Minor Surgical and Nursing Procedures:
      • Leading the development of the nurses' role in the delivery of the service.
    • Mental Health; Learning Difficulties; Physical Disability:
      • Ensuring that appropriate systems are in place for effective patient care and monitoring.

  • Up-to-date training in childhood immunisations
  • Health promotion
  • Family planning

Qualifications

  • RGN Qualification
  • Current NMC registration
  • Accredited training in chronic disease management
  • Cervical cytology qualification
