Position Overview
We are seeking a committed and skilled Medication Management Specialist to become a vital part of our dynamic healthcare team.
The successful candidate will collaborate with a diverse group of healthcare professionals, including General Practitioners, Nurse Practitioners, and support staff, under the guidance of the Practice Manager and Operations Manager.
The ideal applicant will possess a strong background in healthcare, demonstrating flexibility, organization, and a passion for patient care. A recognized qualification as a Pharmacy Technician is essential, along with enrollment or completion of the Primary Care Pharmacy Educational Pathway (PCPEP).
Your responsibilities will encompass a range of tasks, such as managing prescription inquiries, conducting medication reviews, and providing guidance on best practices in medication management.
Key Responsibilities
- Facilitate and document the accurate addition and removal of medications based on discharge summaries and correspondence from secondary care.
- Handle telephone inquiries regarding prescription orders from patients.
- Organize and maintain both repeat and acute medication lists.
- Advise the healthcare team, care facilities, and patients on the administrative aspects of medication management.
- Conduct basic medication reviews and associated health assessments.
- Coordinate requests for medical devices, including catheters and nutritional supplements.
- Provide independent information to patients regarding medication and prescribing changes, initiating further support from healthcare professionals as necessary.
