Position Overview
We are seeking dedicated and compassionate individuals to join our Community Palliative Care Support Team. This role involves providing essential support to patients with life-limiting conditions, ensuring they receive personalized and dignified care in their own homes.
Key Responsibilities
- Deliver care as directed by healthcare professionals for individuals with palliative needs.
- Monitor and assess patients' conditions during visits, promptly reporting any changes to the appropriate healthcare personnel.
- Provide emotional and spiritual support to families and caregivers during challenging times.
- Assist with nutritional needs, ensuring patients receive food and fluids in a manner that respects their preferences and capabilities.
- Maintain oral hygiene for patients to enhance comfort and well-being.
- Engage in hands-on care, demonstrating a commitment to high-quality nursing support.
- Collaborate with the team to ensure a comprehensive service for patients and their families during the final stages of life.
- Support patients and families in coping with disability, loss, and transitions.
- Utilize equipment safely and effectively, following training protocols.
- Document care accurately in electronic patient records, adhering to high standards of record-keeping.

Qualifications and Experience
Essential:

Additional Requirements
A valid driving license is essential for this role, with the ability to use a personal vehicle for work-related travel. Reasonable adjustments may be considered to accommodate individuals with disabilities.
