Position Overview

We are seeking dedicated and compassionate individuals to join our Community Palliative Care Support Team. This role involves providing essential support to patients with life-limiting conditions, ensuring they receive personalized and dignified care in their own homes.

Key Responsibilities

  1. Deliver care as directed by healthcare professionals for individuals with palliative needs.
  2. Monitor and assess patients' conditions during visits, promptly reporting any changes to the appropriate healthcare personnel.
  3. Provide emotional and spiritual support to families and caregivers during challenging times.
  4. Assist with nutritional needs, ensuring patients receive food and fluids in a manner that respects their preferences and capabilities.
  5. Maintain oral hygiene for patients to enhance comfort and well-being.
  6. Engage in hands-on care, demonstrating a commitment to high-quality nursing support.
  7. Collaborate with the team to ensure a comprehensive service for patients and their families during the final stages of life.
  8. Support patients and families in coping with disability, loss, and transitions.
  9. Utilize equipment safely and effectively, following training protocols.
  10. Document care accurately in electronic patient records, adhering to high standards of record-keeping.

Qualifications and Experience

Essential:

  • Understanding of community care principles, gained through education or practical experience.

  • Knowledge of care procedures, including physical care and symptom management for terminally ill patients.

Additional Requirements
