Key Responsibilities

As a member of our established and dedicated team, you will:

  • Collaborate with care coordinators, social prescribers, and health coaches to deliver exceptional patient care.
  • Coordinate and streamline patient appointments across various departments to ensure a seamless experience.
  • Act as a primary contact for patients, families, and community resources, providing information and support with empathy.
  • Assist patients in understanding their care options and facilitate shared decision-making.
  • Maintain confidentiality and uphold patient rights at all times.
  • Develop personalized care and support plans that address individual needs.
  • Support patients in managing their health queries and appointment scheduling.
  • Identify high-risk patients and maintain a comprehensive register of care activities.
  • Conduct visits or arrange appointments for patients as needed, ensuring appropriate clinical assessment and referral.
  • Work closely with social prescribing link workers and other healthcare professionals to provide coordinated care.

Candidate Profile

We are seeking a compassionate and organized individual with:

  • Experience in coordinating patient care.
  • A minimum of 1 year of experience in a primary care or community setting.
  • Strong administrative skills and proficiency in Microsoft Office applications.
  • Exceptional communication skills, with the ability to empathize and support patients effectively.
