We are seeking a dedicated and committed professional to support the engagement of diverse communities within the Black, Asian, and ethnic minority communities who may be experiencing health inequalities and poor health outcomes.
Key Responsibilities- Support the establishment of sustainable links with BAME communities and organizations.
- Develop referral pathways and identify community venues to deliver treatment and interventions in the heart of the community.
- Act as a 'key worker' to a caseload of service users, supporting them to decide their goals around treatment and develop plans to achieve these.
- Provide assessments, risk management planning, and work closely with clinical staff around prescribing needs and partner agencies.
- An understanding of substance and alcohol use and the challenges and opportunities for individuals on a recovery journey.
- Excellent communication skills and the ability to work dynamically on a one-to-one and group basis.
- Knowledge of relapse and other barriers to recovery.
- Flexibility and an ability to work in a fast-paced environment.

- A recruitment and retention allowance of £2,172 - £2,542 paid in addition to basic salary dependant on experience.
