You will engage closely with clients and their families, while also being part of a dedicated team of Healthcare Professionals who are committed to providing compassionate care. Key Details:­

Position: Community Care Evaluator Location: Tameside Work Schedule: Full-time, 35 hours per week, plus on-call duties. Compensation: Up to £23,000 based on experience, including on-call pay and bonuses.

Due to the requirements of this position, a full UK driving licence and access to a vehicle are essential.

What We Are Looking For:

A dedicated individual who is genuinely passionate about delivering high-quality care to our clients. At least 1 year of experience in community care. A solid understanding of the health and social care landscape and the delivery of person-centred care.

Daily Responsibilities:

Crafting care plans and conducting risk assessments. Supervising the safe administration of medications. Performing observations, spot checks, and quality assurance tasks. Maintaining and updating care records in our internal systems.
