Job Title: Support Coordinator

Humankind is seeking a dedicated and compassionate Support Coordinator to join our team. As a key member of our organization, you will play a vital role in providing specialist support to LGBT+ young people and their families.

About the Role

The Support Coordinator will be responsible for managing a caseload of service users, conducting comprehensive assessments and risk assessments, and developing support plans to meet individual needs. You will also deliver support sessions to small groups of parents/caregivers and professionals to increase knowledge and awareness of LGBT+ issues.

Key Responsibilities
  • Manage a caseload of service users and complete high-quality assessments and risk assessments.
  • Develop comprehensive support plans to identify individual needs and goals.
  • Deliver support sessions to small groups of parents/caregivers and professionals.
  • Work collaboratively with the team to provide cross-provision/service support.
