Key Responsibilities:- Lead and manage the nursing team, ensuring effective communication and collaboration with other healthcare professionals.
- Develop and implement clinical policies and procedures, ensuring compliance with best practice and regulatory requirements.
- Provide clinical leadership and guidance to nursing staff, promoting evidence-based practice and continuous professional development.
- Manage and coordinate the delivery of chronic disease management services, including health promotion and disease prevention initiatives.
- Collaborate with other healthcare professionals to develop and implement patient-centered care plans, ensuring seamless transitions and continuity of care.
- Conduct audits and quality improvement initiatives to ensure the highest standards of patient care and service delivery.
- Develop and maintain effective relationships with patients, families, and carers, ensuring their needs are met and their expectations are exceeded.
- Registered Nurse (Nursing and Midwifery Council) qualification.
- Experience of leading a nursing team in a primary care environment.
- Strong clinical skills and knowledge of chronic disease management, health promotion, and disease prevention.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
