Job Title: Lead Practice Nurse

We are seeking an experienced and skilled Lead Practice Nurse to join our team at The Stephenson Medical Practice. As a key member of our primary care team, you will be responsible for leading the nursing team and providing high-quality patient care.

Key Responsibilities:
  • Lead the nursing team and provide clinical guidance and support
  • Develop and implement effective care management programs for patients with chronic diseases
  • Conduct health promotion and wellbeing programs, including QP Autism and Learning Disabilities
  • Assess patient needs and provide appropriate healthcare services
  • Develop and implement health promotion and wellbeing programs
  • Implement and evaluate individual treatment plans for chronic disease patients
  • Identify and manage patients at risk of developing long-term conditions
  • Provide routine nursing care to patients, including wound care, ECGs, and venepuncture
  • Diagnose and manage acute and chronic conditions
  • Prescribe medication for patients in accordance with PGDs and own scope of practice
Requirements:
  • Registered Nurse (Nursing and Midwifery Council)
  • Mentor or teaching qualification
  • Postgraduate diploma or degree in Chronic Disease Management (desirable)
  • Nurse prescriber (desirable)
  • Completed or working towards formal infection prevention and control training (desirable)
  • Family planning qualification (desirable)

  • Meet the requirements and produce evidence for nurse revalidation
  • Evidence of continuing professional development (CPD) commensurate with the role of a Lead Practice Nurse
