Advanced Clinical Practitioner Opportunity at Operose Health

Operose Health is seeking a skilled Advanced Clinical Practitioner (ANP/ACP) to join our team on a part-time basis (12 hours per week). As a Prescriber, you will play a vital role in providing high-quality patient care and contributing to the success of our Primary Care services.
Key Responsibilities- Deliver exceptional patient care, making informed clinical decisions and collaborating with the general practice team to ensure seamless care pathways.
- Develop and maintain strong relationships with patients, families, and healthcare professionals to promote effective communication and coordination of care.
- Participate in multidisciplinary team meetings and contribute to the development of care plans that meet the unique needs of our patients.
- Stay up-to-date with the latest clinical guidelines, research, and best practices to ensure evidence-based care.
- Registered with the Nursing and Midwifery Council (NMC) as an Advanced Clinical Practitioner or working towards non-medical prescriber status.
- Proven experience in a Primary Care setting, with a strong understanding of the challenges and opportunities in this field.
- Excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment.
- Ability to work autonomously and as part of a team, with a strong commitment to delivering high-quality patient care.
