About the Role

We are seeking a highly skilled and experienced Physiotherapist to join our team in the Community Frailty Virtual Ward project. As a Specialist Physiotherapist, you will play a critical role in providing high-quality, patient-centred care to individuals with frailty in our communities across Plymouth, the South Hams, and West Devon.

Key Responsibilities
  1. Work collaboratively with the multidisciplinary team to provide specialist Physiotherapy assessments and interventions for patients admitted to the Frailty Virtual Ward.
  2. Develop and implement individualized Physiotherapy plans to improve mobility, function, and overall well-being of patients.
  3. Provide education and training to patients, carers, and healthcare professionals on Physiotherapy techniques and strategies.
  4. Contribute to the development and implementation of evidence-based Physiotherapy practices and protocols.
  5. Supervise and mentor junior Physiotherapists and other healthcare professionals.
Requirements
  • Postgraduate degree in Physiotherapy or equivalent.
  • Substantial experience in Physiotherapy, particularly in elderly care and community settings.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously and as part of a team.
  • Knowledge of health and social care legislation and policies.
