In this crucial role, you will provide expert clinical care to patients experiencing complex mental health challenges within their homes. Your primary focus will be on delivering timely and effective interventions to support individuals in managing their conditions and achieving optimal well-being.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs, developing individualized care plans that address their unique circumstances.
  • Deliver a range of therapeutic interventions, including cognitive behavioral therapy (CBT), mindfulness techniques, and practical skills training, to empower patients in managing their symptoms and improving their daily functioning.
  • Collaborate closely with multidisciplinary teams, including psychiatrists, nurses, social workers, and other healthcare professionals, to ensure seamless and coordinated care for patients.
  • Provide expert guidance and support to patients' families and caregivers, equipping them with the knowledge and resources to effectively assist their loved ones.
  • Maintain accurate and up-to-date patient records, documenting all interventions and progress made.
Essential Requirements:
  • Registered Occupational Therapist or Physiotherapist with current registration with the relevant professional body.
  • Substantial post-registration experience in a mental health setting.
  • Proven experience of working as an independent practitioner within a multidisciplinary team.
  • Demonstrated ability to conduct comprehensive assessments and develop individualized care plans.
  • Excellent communication, interpersonal, and problem-solving skills.
Desirable Requirements:
  • Experience of working with discharge to assess principles.
  • Additional leadership training and/or experience.
