About The Birches Medical Centre
We are a dynamic and patient-focused medical centre seeking a skilled Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing exceptional patient care and administrative support to our patients and staff.
Key Responsibilities
As a Care Coordinator, your primary responsibilities will include:
- Providing a high-quality patient navigation service, ensuring patients receive the right support at the right time.
- Managing patient records, including demographic information and NHS records.
- Coordinating appointments, visits, and telephone consultations, ensuring accurate information is provided to patients.
- Liaising with local hospitals, pharmacies, and other healthcare professionals as required.
- Processing incoming documents and maintaining patient notes.
- Supporting the practice in developing communication channels between GPs, patients, and other agencies.
- Conducting follow-ups on communications from out-of-hospital and in-patient services.
- Maintaining records of referrals and interventions to enable monitoring and evaluation of the service.
Requirements
To be successful in this role, you will need:
- Excellent organisational skills and ability to work in a fast-paced environment.
- Good communication skills, both written and verbal.
- Strong IT skills, including Microsoft Office and data entry.
- Experience of working in a patient-facing role, preferably in a healthcare setting.
- Ability to work independently and as part of a team.
