About the Role

We are seeking a dedicated and enthusiastic Support Worker to join our team at Polaris Community. As a Residential Childcare Support Worker, you will play a vital role in providing a safe and supportive environment for young people who have been in care and are moving towards independence.

Key Responsibilities
  • Provide responsive and customized support to young people in a residential setting, working in line with person-centered risk assessments and support plans.
  • Observe and document any changes in behavior, ensuring that the home environment remains safe and welcoming for all residents.
  • Maintain positive working relationships with young people, staff, and professionals, communicating effectively and actively seeking ways to improve the experience of young people in our service.
  • Ensure continuous support is provided to young people, adhering to the standards of day staff and prioritizing the safety of vulnerable individuals.
  • Carry out health and safety actions, including cleaning tasks, and adhere to established policies and procedures.
  • Liaise with external agencies, such as the police and Emergency Duty Team, as necessary.
  • Support young people who require hospital or police station visits.
Requirements
  • Experience in supporting young people who may be vulnerable.
  • Flexibility in availability, as the role may include weekend work.
  • Hold a Residential Childcare/Families Practitioner Diploma, Children and Young People's Workforce/NVQ Level 3 in Health & Social Care, or equivalent qualification.
  • Be willing to travel, with a full driving license and access to a vehicle.
  • Excellent computer skills.
