About the Role

We are seeking a highly motivated and dedicated Social Worker to join our team at Wigan Council. As a Social Worker, you will play a vital role in supporting vulnerable adults and their families, providing a range of services to promote their well-being and independence.

Key Responsibilities
  • Assess and identify the needs of service users, developing and implementing personalized care plans to meet their needs.
  • Work closely with service users, their families, and other professionals to provide a comprehensive and coordinated service.
  • Conduct home visits and assessments to identify the needs of service users and develop care plans.
  • Provide support and guidance to service users and their families, promoting independence and well-being.
  • Work collaboratively with other teams and agencies to ensure a seamless service delivery.

Requirements

To be successful in this role, you will need:

  • A degree in Social Work or a related field.
  • Registration with the Health and Care Professions Council (HCPC).
  • Experience of working with vulnerable adults and their families.
  • Excellent communication and interpersonal skills.
  • A commitment to delivering high-quality services to our service users.
