Job Summary:
We are seeking a highly skilled and dedicated Salaried General Practitioner to join our dynamic team at Southern Health NHS Foundation Trust. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and contributing to the development of our services.
Key Responsibilities:
- Provide autonomous patient care and make informed decisions regarding treatment plans.
- Consult with patients through various channels, including face-to-face and remote consultations.
- Develop and implement comprehensive care plans, providing health education and support to patients.
- Refer patients to appropriate care providers and services, ensuring seamless transitions and continuity of care.
- Comply with clinical governance policies and procedures, maintaining the highest standards of patient safety and care.
- Assist in managing chronic medical conditions, working collaboratively with multidisciplinary teams to develop and implement effective care plans.
- Maintain accurate and up-to-date medical records, ensuring compliance with regulatory requirements.
- Meet professional learning requirements, staying up-to-date with the latest clinical guidelines and best practices.
Requirements:
- Fully qualified GMC registered General Practitioner with a strong track record of providing high-quality patient care.
- Previous General Practice training and experience, with a focus on primary care and community-based services.
- Enhanced CRB check and CPR certificate, demonstrating a commitment to patient safety and well-being.
Desirable Skills and Qualities:
- Excellent communication and leadership skills, with the ability to work effectively with multidisciplinary teams.
- Strong organizational and time management skills, with the ability to prioritize tasks and manage competing demands.
- Computer literacy skills, with experience of using electronic patient records and other clinical systems.
