We are seeking an experienced Care Coordinator to join our team, responsible for coordinating the delivery of a safe and well-planned domiciliary care service.

Key Responsibilities:

  • Coordinate staff rotas using a computer-based roster system to ensure all visits are covered.
  • Liaise with external agencies, such as hospitals, GPs, and social services, as well as service users' family members and advocates.
  • Manage a team of community carers, ensuring they have the necessary support and resources to deliver high-quality care.
  • Proactively identify and address any issues or concerns, working closely with the care team to resolve them.

Requirements:

  • Admin background with experience in care and support.
  • Good IT skills and ability to work within a fast-paced environment to deadlines.
  • Understanding of the health and social care sector.
