About the Role

We are seeking a dedicated and compassionate Support Practitioner to join our team at Cornerstone. As a Support Practitioner, you will play a vital role in supporting individuals with disabilities and additional support needs to live independently in their shared home.

Key Responsibilities

  • Support individuals with all aspects of their daily living, including shopping, finances, meal preparation, housework, and personal care.
  • Encourage and support individuals to engage in their hobbies and interests, such as cinema, theatre, football, and creative activities.
  • Work collaboratively with the team to provide a supportive and inclusive environment.
  • Develop and maintain positive relationships with individuals, their families, and other stakeholders.
  • Contribute to the development and implementation of individual support plans.

About You

We are looking for someone who is caring, compassionate, and able to work well in a team. You will have good communication and organisational skills, a positive attitude, and a willingness to learn. You will also have confidence in the use of computers and the ability to cook.

Shift Patterns

Shifts will usually fall between hours, with sleepovers, weekends, and public holidays forming part of the rota on a rotational basis. You will work around 2-3 sleepovers per month, with an additional payment of £96 for each sleepover shift worked.
