Registered General Nurse – Community Services Team

We are seeking a motivated and enthusiastic Registered General Nurse to join our Community Services Team at Woking & Sam Beare Hospice. As a key member of our team, you will play a vital role in delivering high-quality palliative care services to patients and their families in the comfort of their own homes.
About the Role:- Deliver palliative care services to patients with life-limiting illnesses in their own homes.
- Work closely with our multidisciplinary team, including Clinical Nurse Specialists, Associate Clinical Nurse Specialists, Paramedics, and Specialist Doctors.
- Develop and maintain strong relationships with patients, families, and carers to ensure seamless care coordination.
- Participate in regular team meetings and contribute to the development of care plans and strategies.
- Collaborate with other healthcare professionals to ensure comprehensive care and support.
