Proactive Care Matron

We are seeking an experienced and skilled Community Matron to join our team at Surrey Downs Health and Care. As a Community Matron, you will play a key role in delivering proactive care and support to individuals with complex needs, working closely with community GPs, paramedics, pharmacists, district nurses, adult social care, and voluntary organizations.
Key Responsibilities- Provide clinical triage and assessment for inbound referrals, assigning urgency and determining the best clinician for the visit.
- Support annual Learning Disability or Serious Mental Illness Health checks.
- Care coordination, managing patient caseloads, and offering proactive support for frequent attenders, cancer patients, and those at risk of cardiovascular disease.
- Anticipatory care, liaising with primary, secondary, and community teams to reduce admissions for ambulatory care sensitive conditions and close the hypertension diagnosis gap.
- Enhanced Health in Care Homes, proactive and targeted visits for specific cohorts of patients.
- Support and manage a small team of non-clinical care co-ordinators.
- Demonstrate clinical expertise and act as a professional role model, working as part of an integrated team to develop services in line with patient needs.
