About the Role
We are seeking a dedicated and experienced Registered Nurse to join our team, providing specialized care to individuals with learning disabilities and forensic mental health needs within the community setting of Staffordshire. This role offers a unique opportunity to make a real difference in the lives of vulnerable individuals, supporting their well-being and promoting their independence.
Key Responsibilities
- Conduct comprehensive assessments of service users' physical and mental health needs, developing individualized care plans that address their unique requirements.
- Administer medications and treatments as prescribed, ensuring adherence to best practices and safety protocols.
- Provide direct support to individuals with learning disabilities and forensic mental health needs, assisting them with daily living activities, personal hygiene, and social interaction.
- Collaborate effectively with multidisciplinary teams, including doctors, psychologists, social workers, and other healthcare professionals, to ensure coordinated and holistic care.
- Implement risk assessment and management strategies designed to effectively manage and mitigate risks associated with individuals with complex needs.
- Deliver training for staff in community services to enhance their skills in meeting the complex clinical needs of individuals with learning disabilities and forensic needs.
- Develop and implement teaching materials and training packages that promote best practice within the field.
- Facilitate access to forensic services and inpatient admission when necessary, providing guidance on suitable reasonable adjustments and support implementation within learning disability and forensic services.
- Work collaboratively with colleagues from local authorities and other health providers to ensure timely discharge to appropriate community-based services, adhering to all locally agreed protocols, including CPA and 117 Aftercare entitlements.
- Case manage a small number of individuals with complex needs who present a high risk of offending or admission to an inpatient setting (identified from the Dynamic Risk Register).
- Engage in Collaborative Safety Planning (and other interventions) with service users on your caseload, aiming to reduce the risk of offending behavior.
Essential Requirements
- Qualified RNLD Nurse.
- Current professional registration or accreditation with the appropriate body for qualified role (e.g., NMC).
- Relevant forensic qualification/experience.
- Experience of working with people with a Learning Disability and forensic needs in community and/or hospital settings.
- Experience of working within multidisciplinary teams and as an autonomous practitioner.
- Experience of managing and prioritizing a complex caseload.
- Knowledge of Learning Disabilities, Physical Health, Positive Behaviour Support, Applied Behaviour Analysis, and Forensic Practice.
- Knowledge of current DOLS, Mental Capacity, and Mental Health Act legislation.
- Ability to practice ethically, considering the needs of the person receiving care and their best interests.
