We're seeking a talented individual to join our team as Home First System Lead, playing a crucial role in shaping the future of our services.

The Role

This is a fantastic opportunity for an experienced Occupational Therapist or Social Worker to lead the development of a county-wide service supporting people to live independent lives in the community. As Home First System Lead, you will work closely with domiciliary reablement providers and the in-house multi-disciplinary team to deliver high-quality reablement services for people leaving hospital and those already in their own homes.

Key Responsibilities
  • Lead the implementation of Discharge to Assess pathways, ensuring patients are discharged within 48 hours of being medically optimized.
  • Develop community resources to maximize opportunities for return to independence.
  • Build and maintain partnerships with sector organizations to deliver a multi-agency model for Discharge to Assess delivery.
  • Collaborate with the out-of-hospital Discharge to Assess team, including social workers, therapists, discharge coordinators, third sector, and provider partners.
  • Drive improved patient and system outcomes, as well as financial efficiencies, through increased independence and reduced hospital stays.
