About the Role

We are seeking a highly skilled and experienced NHS Continuing Healthcare Specialist to join our team at NHS Cambridgeshire & Peterborough. As a key member of our Integrated Care System, you will play a vital role in ensuring high-quality care for our patients.

Key Responsibilities
  • Provide expert advice on NHS Continuing Healthcare, NHS-funded Nursing Care, and joint-funded care to clients, health and social care professionals, and other agencies.
  • Lead and coordinate the NHS CHC assessment process, including carrying out nursing assessments and MDTs.
  • Liaise with clients and/or their representatives, health and social care professionals, and other agencies to source suitable placements and/or packages of care.
Requirements
  • Registered Nurse - General/Adult/Mental health/Learning Disability or Allied Health Care Professional (Occupational therapist/physio therapist, speech and language therapist).
  • Valid Professional registration.
  • Continuing Care Specialist training or qualification.
Experience
  • Experience of carrying out complex nursing/healthcare assessments for clients with health and social care needs.
  • Experience of care planning for clients with complex health and social care needs.
  • Experience of working in partnership between health and social care.
Skills, Ability & Knowledge
  • Awareness of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care.
  • Knowledge and understanding of NHS and Social Care Services structures and Independent Sector Organisations.
  • Knowledge of the Care Act 2014 and its implementation.
Working Arrangements

This is a full-time position with flexible working arrangements. The role is predominantly home-based, with some roles requiring occasional travel to meet with clients and/or their representatives.
