We are seeking an experienced and skilled Community Practitioner to join our Personality Disorder Hub Team at Cumbria, Northumberland, Tyne and Wear (CNTW) NHS Foundation Trust.
Key Responsibilities- Provide specialist care coordination and treatment for individuals with emotionally unstable personality disorder diagnosis, co-existing conditions, and high risk.
- Implement Structured Clinical Management (SCM), adapted trauma stabilization, and Mentalisation Based Treatment (MBT) during the most unstable phase of a service user's care pathway.
- Focus on providing safety, stabilization, containment, and emotional regulation and control.
- Scaffold a wide range of community teams, inpatient staff, and third sector providers.

We welcome applications from qualified professionals with suitable experience, including Registered Mental Health Nurses, Occupational Therapists, and Social Workers.
Person Specification- Professional Qualification and current Professional Registration as a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker, or Clinical Psychologist.
- Evidence of post-registration training in clinical/psychological interventions/skills relevant to the post.
- Proven track record of post-registration clinical experience relevant to the post.
- Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs.
- Risk Assessment and Risk Management/Safety Planning skills.
- Demonstrate an understanding of effective and appropriate treatment options, including a range of evidence-based clinical approaches.
- Demonstrate effective written and verbal communication skills.
- Awareness and understanding of professional accountability, NMC standards, and other local and national initiatives.
