We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will be responsible for providing high-quality mental health services to patients across a range of settings.

Key Responsibilities

  • Assess and support individuals with mental health needs, including those with complex and high-risk presentations.
  • Develop and implement comprehensive care plans, working closely with patients, families, and other healthcare professionals.

Requirements

  • Professional registration in health or social care, with up-to-date accreditation and CPD.
  • Experience working in acute mental health services, with a strong understanding of assessment tools and mental health conditions.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
  • Ability to work autonomously, prioritize tasks, and manage workload effectively.
