About the Role
We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Pennine Care NHS FT. As a key member of our multidisciplinary team, you will be responsible for providing high-quality mental health services to patients across a range of settings.
Key Responsibilities
- Assess and support individuals with mental health needs, including those with complex and high-risk presentations.
- Develop and implement comprehensive care plans, working closely with patients, families, and other healthcare professionals.
- Provide expert advice and guidance to colleagues on mental health-related matters.
- Contribute to the development and implementation of service improvements, ensuring high-quality patient care and outcomes.
- Work collaboratively with other disciplines to ensure seamless care transitions and effective communication.
Requirements
- Professional registration in health or social care, with up-to-date accreditation and CPD.
- Experience working in acute mental health services, with a strong understanding of assessment tools and mental health conditions.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and colleagues.
- Ability to work autonomously, prioritize tasks, and manage workload effectively.
- Strong organizational, problem-solving, and time management skills, with a positive attitude to change and commitment to service development.
