About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in Leeds. As a Support Worker, you will play a vital role in providing advice and support to Care Experienced Young People aged 18-25 within supported living accommodation.

Key Responsibilities
  • Provide emotional and well-being support to young people, helping them to access key services such as housing, welfare benefits, health, education, and employment.
  • Work closely with partner agencies, including Social Care, Housing Providers, Job Centre, and other mainstream providers, to ensure a comprehensive support network for our young people.
  • Develop and maintain strong relationships with young people, understanding their individual needs and goals, and providing tailored support to help them achieve their aspirations.
  • Contribute to the development and implementation of support plans, ensuring that young people receive the best possible outcomes.
