Physiotherapist - Community Care Specialist

We are seeking a highly skilled and dedicated Physiotherapist to join our Community Team at Wiltshire Health and Care LLP. As a Specialist Physiotherapist, you will be responsible for delivering high-quality therapy care to patients within our community setting.
Key Responsibilities- Provide highly skilled therapy care to patients within our geographical area, promoting the health of the population, reducing the effects of ill health and disability, and maximizing the individual's potential for independence.
- Work closely with General Practices, Social Services, and other statutory and non-statutory services to meet the needs of the local population, requiring excellent communication skills.
- Support the development, implementation, and maintenance of high-quality safe and sustainable clinical and operational leadership under the direction of the team leaders and clinical lead.
- Participate in the weekend therapy rota and cover planned and urgent occupational therapy and physiotherapy assessments and treatment.
- MCSP Dip PT / BSc / BSc(Hons) PT / MSc Physiotherapist
- Registered as a Physiotherapist with the Health and Care Professionals Council
- Relevant postgraduate qualification
- Up-to-date community experience, case management experience, experience of working with adults, and working with a multi-disciplinary team
- Leadership experience, teaching experience, knowledge of recruitment, selection, and appraisal systems, and setting and monitoring standards of care
- Non-medical prescriber
- Leadership and Management training
- Further qualifications relevant to community practice
- Profiling community needs and targeting identified health needs of the population
- Evidence of innovative and flexible approach to care and the organization of care
- Access to a vehicle that can be used for business purposes and hold a full and valid driving license
