Physiotherapist - Community Rehabilitation

We are seeking a highly motivated and experienced Physiotherapist to join our Community Rehabilitation Team in Carlisle & District ICC. As a key member of our team, you will play a vital role in delivering high-quality patient care and rehabilitation services to the local population.
The successful candidate will have a strong background in physiotherapy, with experience of working in a community setting and a willingness to work towards the competency framework for a Band 6 Senior Physio role. You will be expected to work closely with members of the ICC, Adult Social Care, and other agencies to provide patient-centred support and rehabilitation plans.
The main duties of the job include:
- Prevention of unnecessary hospital admissions and facilitation of early discharge from the hospital setting
- Provision of patient-centred support and rehabilitation plans
- Participation in the Lead Professional of the Day rota, Discharge to Assess, and rapid response functions of the team
- Delivery of group-based rehabilitation in patients' homes, hospital gym settings, and community hospital wards
- Supporting physiotherapy students on placement and contributing to the ongoing learning and development of staff within the team and local MDT
The post holder will be expected to work flexibly, including late shifts, weekend working, and Bank Holidays. A full clean UK driving licence and access to own transport are essential.

Person SpecificationThe ideal candidate will have:
- Completion of core junior rotations (Respiratory/neurological/musculo-skeletal/elderly care)
- Evidence of Continuing Professional Development
- Experience of working within the NHS
- Previous experience of working at a Band 6 level
- Experience of working within a community setting
- Experience of working within a multi-disciplinary team
- Teaching/Mentoring of healthcare professionals
The successful candidate will hold:
- CSP recognised Degree/Diploma or equivalent in Physiotherapy
- HCPC registered
- Evidence of attendance at relevant postgraduate courses
- Member of the Chartered Society of Physiotherapy
The ideal candidate will possess:
- Good knowledge of Evidence-Based Clinical Practice within Rehabilitation
- Good knowledge of Continuing Professional Development/Lifelong Learning
- Knowledge and understanding of Clinical Governance including Risk Management
- Understanding of Professional Code of Conduct and confidentiality
- Knowledge of key NHS drivers for change and implications for Physiotherapy services
- Knowledge of management of patients with long-term conditions and falls
- Knowledge of clinical audit and outcome measures, and their use in service improvement
- Ability to communicate effectively
- Ability to motivate self and others
- Relevant practical physiotherapy skills
- Good IT skills, including data input, email communication, and searching for evidence-based practice
The successful candidate will be able to:
- Travel independently, including transporting patients and/or equipment across the locality
- Work flexibly, including late shifts, weekend working, and Bank Holidays
- Be reliable and friendly, with a positive approach to work
The post holder will be expected to:
- Take an active part in training for personal and professional development
- Be willing to actively take part in training of others
- Have a full clean UK driving licence and access to own transport, including business insurance
- Develop their role from Band 5 to Band 6 position using AHP developmental framework
