About the Role

We are seeking a dedicated and compassionate Support Worker to join our team in Leeds. As a Support Worker, you will play a vital role in providing advice and support to Care Experienced Young People aged 18-25 within supported living accommodation.

Key Responsibilities
  • Provide emotional and well-being support to young people, helping them to access key services and achieve their goals.
  • Work closely with partner agencies, including Social Care, Housing Providers, and Job Centre, to ensure seamless support for our young people.
  • Develop and maintain strong relationships with young people, understanding their individual needs and circumstances.
  • Contribute to the development and implementation of support plans, ensuring that young people receive the best possible care and support.

Requirements

  • Knowledge of the issues affecting Care Experienced Young People and the ability to provide tailored support.
  • A commitment to delivering high-quality support and achieving positive outcomes for our young people.
