Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our Primary Care Network team. As a Clinical Pharmacist, you will play a vital role in providing support to our practices with regards to medication and prescription queries.

Main Responsibilities
  • Provide expert support to practices with medication and prescription queries.
  • Support the repeat prescription process, addressing both medical and social needs of patients.
  • Contribute to chronic disease management, medication reviews, and proactive management of patients with complex polypharmacy.
  • Be actively involved in the management of patients with long-term conditions.
  • Manage medication reviews and safe and effective prescribing in care homes.
  • Provide patient-facing and telephone support to patients with queries and concerns regarding their medication.
  • Implement changes required regarding MHRA alerts and local and national guidance.
  • Provide advice and recommendations on shared care, NICE guidance, and RAG lists.
  • Manage common and minor ailments within the limits of your capability.
  • Manage discharge summaries from hospitals and community services.

Person Specification
  • Completion of an undergraduate degree in pharmacy and registration with the General Pharmaceutical Council.
  • Minimum of 2 years experience as a pharmacist, demonstrated with a practice portfolio.
  • Experience and awareness of common acute and long-term conditions likely to be seen in general practice.
  • Ideal candidate will hold or be working towards an independent prescribing qualification.
  • Able to follow legal, ethical, professional, and organisational policies/procedures and codes of conduct.
