About the Role

The Norfolk & Suffolk Foundation NHS Trust is seeking a skilled Mental Health Practitioner to join our Community Mental Health Recovery Team. As a key member of the team, you will play a vital role in providing assessment, treatment planning, and interventions to service users with moderate to severe mental illness.

Main Responsibilities

  • Conduct comprehensive assessments of service users to identify their needs and develop personalized treatment plans.
  • Provide evidence-based interventions to support service users in achieving their recovery goals.
  • Collaborate with multidisciplinary teams to ensure seamless care coordination and effective risk management.
  • Develop and maintain strong relationships with service users, their families, and carers to promote a supportive and inclusive environment.

Requirements

  • Current Professional qualification as a Registered Nurse, Occupational Therapist, or Social Worker.
  • Proven experience in mental health practice, with a focus on community-based care.
  • Excellent assessment and risk management skills, with the ability to work effectively in a fast-paced environment.
  • Strong communication and interpersonal skills, with the ability to build rapport with service users, colleagues, and other stakeholders.
