As a Personal Advisor, you will play a crucial role in the Early Help & Children's Social Care sector, ensuring that children and families receive timely and effective support. Our mission is to create a safe and nurturing environment for children, enabling them to thrive and reach their full potential.
- Fulfill the responsibilities of a Personal Advisor in accordance with relevant legislation.
- Provide guidance to care leavers as they navigate independent living.
- Collaborate with various agencies to ensure high-quality services for young people.
Your responsibilities will include:
- Delivering a professional social work service within a multi-agency framework, assessing the needs of children and families.
- Advising young individuals in care on their Transition Plans for independent living.
- Maintaining regular communication with care leavers to assist them in developing essential life skills.
- Advocating for young people with external agencies and helping them build negotiation skills.
- Supporting young individuals in fostering relationships with family and peers.
- Regularly reviewing and updating Pathway Plans for young people.
- Documenting interactions and maintaining records in line with organizational policies.
- Gathering feedback from young people to enhance service delivery.
- Engaging with other teams and participating in inter-agency discussions.
- Contributing to team development and sharing expertise with colleagues.
- Participating in supervision and personal development planning.
- Assisting in coverage for absent colleagues as needed.
- Adhering to the requirements of relevant legislation.
- Performing additional duties as directed by management.
