About the RoleThis is an exciting opportunity to work in a dynamic and evolving service, providing assessment, treatment, and care support to patients in dedicated bed-based units or in their own homes. You will be required to develop skills that will support all areas within the Home First Rehabilitation pathways.
Key Responsibilities- Provide specialist physiotherapy assessments over a range of conditions and complexities, using advanced clinical reasoning to analyse clinical and non-clinical findings.
- Assess patient understanding of assessment and treatment, and assist members of the team to work within the legal framework for patients who lack capacity to consent to treatment.
- Provide health education, health promotion, and prevention advice.
- Formulate and recommend the best course of physiotherapy intervention and/or treatment programmes to meet the agreed goals set with the individual patient/or carer.
- Use a range of verbal and non-verbal communication methods to communicate effectively with patients and carers to maximise rehabilitation potential.
- Overcome barriers to appropriate rehabilitation, including communication difficulties, anxiety, or motivation.
