We are seeking a dedicated Healthcare Support Specialist to join our team at Maryport Health Services. The position entails working collaboratively with a team of Care Coordinators, Social Prescribers, and other healthcare professionals, with a commitment of up to 30 hours per week.
The ideal candidate will possess experience in patient-facing roles within the healthcare sector, particularly in the NHS. The primary objective of this position is to empower individuals to maintain their health and well-being, enabling them to live independently in their homes for as long as possible.
The successful applicant will deliver a comprehensive, integrated service to those identified as moderately frail and not currently receiving a care plan.
Key Responsibilities
The Healthcare Support Specialist will manage a short-term caseload, ensuring that individuals receive appropriate care and support while addressing any unplanned changes in their needs. Responsibilities include:
- Case Identification: Proactively identify and engage with patient groups within the moderate frailty category, utilizing relevant search tools as necessary.
- Assessment: Conduct holistic, person-centered assessments in collaboration with patients, their families, and caregivers, ideally in the patient's home environment.
- Action Planning: Develop and implement tailored action plans that address both short-term and medium-term needs.
- Coordination: Liaise with various statutory and voluntary organizations to ensure patients have access to a broad spectrum of support services.
- Monitoring: Regularly review and adjust care plans in partnership with patients to evaluate outcomes and make necessary changes.
Communication and Collaboration
The role requires effective communication with patients, families, and healthcare teams. The Specialist will serve as a central point of contact, fostering strong relationships with all stakeholders involved in the care process.
Administrative Duties
The position includes maintaining accurate electronic patient records and ensuring timely documentation is shared with relevant professionals.
