We are seeking a highly skilled and compassionate Enhanced Care Recovery Coordinator to join our Forward Leeds Team. As a key member of our multi-disciplinary recovery team, you will play a vital role in supporting adults and young people struggling with alcohol and other drugs through dedicated prevention, early intervention, and tailored programs.
Key Responsibilities- Assess and engage with individuals identified to have multiple needs, providing holistic support to facilitate their drug and alcohol treatment.
- Manage a service user caseload, reviewing ongoing care and treatment, and liaising closely with specialist workers within the multi-agency team.
- Conduct home visits, support clients to appointments, and meet individuals in the community to provide flexible and effective support.
- Exercise excellent communication and therapeutic skills when dealing with service users, carers, and caring agencies.
- Work closely with the multi-disciplinary team, providing advice and guidance to managers and colleagues through attendance at reviews, team meetings, and group supervision.
We are looking for a passionate and dedicated individual with a full clean UK driving license and access to their own vehicle. You will have a strong understanding of interventions, including brief interventions, motivational interviewing, psychosocial interventions, cognitive behavioral therapy, harm reduction, and multi-disciplinary working.
